Default Calculating fair value?

Not sure about this but is there an index that measures fair value of the S&P 500 that I can graph to see when program trading maybe occuring?

Hmm .. not sure. May be contact your preferred data provider if they support some index related?

Default Fair Value.. etc..

I think for TS data it is $ESINX and for ESignal data it is $EPREM Those are both just for the S&P 500.

Then you can go over to www.indexarb.com , scroll down to the third graph, which is titled: Our Trading Guidelines For (date): and jot down the buy and sell targets, post those as horizontal alert lines on your new symbol chart at least a tick or two below the buy and a tick or two above the sell and there ya go. If you have DTN or some other data provider, you will have to check with them for the proper symbol for the index to monitor.

Default Fair Value Premium Index indicator

yes, some data providers already providing fair value premium indices for ES NQ and TF.
DTN for sure. eSign only provides $EPREM for ES / SPX.X The formula, however, is simple. and I made my way to this forum hoping someone might have already written a custom indicator for it. (^RUT - TF 06-12 = index). Assumes subscriber has real time ^RUT in data vendor subscription.....

also looking for a ratio Advance / Decline indicator line plot. Will start a separate thread for this one, but presume is basically the same, and would assume vendor can supply real-time NYSE Advance and Decline numbers separately...

You should be able to accomplish with any of the "index" or "spread" indicators in our file sharing section. Please take a look at this filtered list and see if any of these will do what you're looking for:
