Order Rejected by IB

Hi guys. Running live today for the first time.

I have 3 instances of my strategy running. When the third strategy decided to place an order, it was rejected by IB. [The first 2 had already placed orders with no problem.] Relevant log entries:

I can access this symbol through TWS (I spent a round trip commission to be sure), so I'm sure it's not my account. I also tried to submit through the standard order interface outside of a strategy with the same results. It must have something to do with my Instrument Manager set up I guess? Does NT always send orders to the SMART destination at IB? I tried with both Nasdaq and Default selected, same result.
