Stop not filled?

Hello,

I had a buy stop order that was not filled this morning, and I'm not sure why. I actually had two stop orders, one to buy 4 contracts, and another buy stop one tick higher for 2 contracts. When the market moved on high volume, my stop for 4 contracts was filled but my stop for 2 contracts was NOT filled, it was still sitting there untouched after the market had moved through the price and beyond. That stop not getting filled cost me about $1000 in missed profit, so I'm trying to figure out why it was not activated. I contacted the IT desk at my FCM and they said to contact NT support about the issue, please advise.

Was this is a stop market or a stop limit order? If a stop limit, what was the limit offset used?

What time was the order put in in terms of when it should have filled? Was it less than 30 seconds before?

It was a stop market order, not a stop limit order, and yes it was placed less than 30 seconds before the move. I'll shoot you guys an email so I can get the log files over.
